“The button assembly on a limited number of 1.1” Omni Swivel Pulleys and associated variants may not be secure. If the button comes out the side plate can open, allowing the rope to fall out which can result in serious injury or death. It was discovered that a small number of pulleys manufactured from February 2020 through October 2022 may not have had adequate thread locker or sealant, allowing the set screw to back out. Under normal circumstances, either the thread lock or sealant is adequate to secure the button and prevent this issue.
Rock Exotica is issuing a recall for Omni Block 1.1” style pulleys, manufactured from February 2020 through October 2022. These pulleys must be returned to us for inspection / repair.”

“Climbing Technology has voluntarily issued a request for inspection of the ROLLNLOCK pulley/rope clamp (Ref. No. 2D652) by the user. One of our customers has detected a defect in the functioning of the “sheave position lever” (F). This defect can lead to an accidental locking of the sheave position lever on its pin (H) on the plate. As a result of this defect, when using the device in “hauling pulley” mode, the lever may inadvertently engage the pin suddenly, disabling the locking function and releasing the load or person. “

As part of 3M Fall Protection’s ongoing commitment to delivering high quality safety equipment, we are notifying our customers of the following information related to the 3M™ DBI-SALA® Delta™, ExoFit™, ExoFit™ Plus & ExoFit™ XP Cross Over Style Harnesses portfolios. During internal testing of certain 3M™ DBI-SALA® Delta™, ExoFit™, ExoFit™ Plus & ExoFit™ XP Cross Over Style Harness models, we found that a percentage of tested units did not meet the ANSI Z359.11, Section 3.2.2.3.1 dynamic requirement on the front (sternal) D-ring. This test and the resulting forces are not representative of what would be seen in real world application (i.e. when attached to a vertical cable sleeve with an energy absorbing device).”

An improperly formed rivet may become displaced from the top swivel eye. An unformed rivet may result in the SRL becoming detached from the anchor hook, which could result in severe injury or death.”

In all tests, these lanyards continued to meet all dynamic requirements of OSHA 1926.502 (d)(16) proving that these lanyards would arrest a fall. The Pro™ Pack Twin Leg Shock Absorbing Lanyard portfolios are also certified to ANSI Z359.13 and meet the dynamic requirements of this standard. We are not aware of any accidents or injuries related to the condition described above. We have not received any customer complaints regarding this but have identified it through an internal review.”

Security Alert: Wind Escapettor Rescue Descender and Parttner Complement
We have detected that when the following conditions are met at the same time:
1. There is an overload in the equipment.
2. An excessively strong actuation of the lever is carried out.
The Wind Escapettor rescue descender can damage the rope, even severing it due to the aggressiveness of the anti-panic cam tooth of some equipment. This problem has been identified in the devices with the serial number starting by 20.05 and 20.02 These circumstances of unforeseen use, in which the rope can be damaged, are exceptional and do not correspond to the usual use of the equipment, nor to those contemplated in the tests carried out for its approval.

User Safety Notice: Latchways Standard Self-Retracting Lifeline
MSA is issuing this User Safety Notice to inform you that MSA has received field reports of a limited number of Latchways Standard Self-Retracting Lifelines (SRL) in which some internal bolts came loose. MSA has not received any reports of injuries associated with this condition. However, we are requesting that you perform the actions outlined in this notice. Over time, the loose bolts will be identifiable by the user as extraction of the cable from the SRL housing will no longer be possible. However, MSA’s investigation has determined that this may be preceded by a window of time in which a fall may not be arrested.

Stop Use & Product Recall: 3M™ DBI-SALA SELF-RESCUE DESCENT SYSTEM
“Given this recent information, 3M Fall Protection is issuing a “Stop Use and Recall” of all 3 models of the 3M™ DBI-SALA® Self-Rescue Descent Systems including P/N 3320030; 3320031 & 3320037. We have completed a review of returned training units from the field and have concluded that this cracking condition is also a result of embrittlement overtime of the plastic spool used to hold the lifeline rope. This embrittlement could inhibit the ability of the Self-Rescue Descent System to fully descend an end-user in the event of a fall. This spool is internal to the Self-Rescue Descent System and unlike the training, models cannot be inspected. Please note we are not aware of any accidents or injuries related to this condition.”

With respect to the voluntary ANSI/ASSE Z359.13-2013 standard, recent 3M testing produced results marginally (~2-4%) above the maximum requirement for testing in ambient, dry conditions.1 We believe this is the result of a raw material issue, which we are currently investigating (with in-scope product shipments currently on hold pending completion of that work). STOP USE & PRODUCT RECALL 3M™ DBI-SALA® Nano-Lok™ edge and Wrap Back Twin-Leg Self Retracting Lifelines
“3M Fall Protection announces an immediate stop use and product recall of the 3M™ DBI-SALA® Twin-Leg Nano-Lok™ edge and the Twin-Leg Nano-Lok™ Wrap Back Self-Retracting Lifeline. These Twin-Leg Nano-Loks are used as part of a personal fall protection system that connects two self-retracting lifelines/devices (SRLs/SRDs) directly under the dorsal D-ring of a worker’s harness. The Twin-Leg Nano-Lok edge is intended to be anchored at foot-level, is designed for sharp and/or leading edge applications and incorporates an energy absorber. The Twin-Leg Nano-Lok Wrap Back is intended for wrapping around an anchor and incorporates a similar energy absorber. 3M has determined that in a fall and under certain conditions, the energy absorber of these devices may not properly deploy which could expose the worker to serious injury or death. There have been no reports of accidents or injuries associated with this issue, these products must be removed from service immediately.
To address this situation, in the interests of worker safety, 3M is launching a global stop use of the Twin-Leg Nano-Lok edge and Twin-Leg Nano-Lok Wrap Back SRL devices. 3M is recalling all these devices to be repaired or replaced as soon as a solution is identified, tested and certified. Alternatively, end-users may elect to receive cash for their returned devices as described below. This stop use/recall affects all models of the Twin-Leg Nano-Lok edge and Twin-Leg Wrap Back SRLs since first introduced in 2013. This stop use/recall does not affect the single-leg versions of these models. As of the date of this notice, no 3M twin-leg SRL product is approved for use over sharp or abrasive edges.“

Stop Use & Product Recall IMMEDIATE ACTION REQUIRED 3M™ PROTECTA® Cobra Rope Grab AC202D
“3M Fall Protection has determined that the 3M™ PROTECTA® Cobra Mobile/Manual Rope Grab AC202D is incorrectly stamped for use with 1/2 to 3/4 inch diameter rope on the exterior body of the unit. This Cobra Rope Grab is certified for use ONLY with 5/8 inch diameter polyester or polypropylene rope. All marketing information and the Instructions for Use (IFU) for this rope grab correctly identify the size of rope to be used as 5/8 inch polyester/polypropylene rope.
In the event of a fall from height, a Cobra Rope Grab used with 1/2 inch diameter rope may not arrest the fall and could result in serious injury or death to the worker. To remedy this situation, 3M is launching a Stop Use & Product Recall to replace all AC202D and related Cobra Rope Grabs that are stamped 1/2″—3/4″ Dia.”

“3M Fall Protection has learned of the possibility of a manufacturing defect in a dorsal d-ring utilized in ExoFit NEX™ harnesses manufactured between January 2016 and December 2018. Although there have been no reported incidents involving this condition, a dorsal d-ring with this defect will not support the load in a fall arrest event which could result in serious injury or death. Harnesses manufactured only within this date range require immediate inspection for lot number 09P1 stamped into a dorsal d-ring. We believe that only one harness was manufactured with a defective D-ring, but we urge inspection of all potentially affected harnesses out of an abundance of caution in the interests of worker safety.
To address the situation, 3M Fall Protection is launching a global Inspection Notice. As set out in the attached Notice, all ExoFit NEX™ harnesses manufactured between January 2016 and December 2018 require inspection prior to use.”

QUALITY ALERT – 3M™ DBI-SALA® Exofit™ Harnesses
“3M Fall Protection has received reports that the aluminum reinforcement plate used in the seat sling on certain DBI-SALA ExoFit™, ExoFit XP™ (inc. Arc Flash), and ExoFit NEX™ (inc. Arc Flash) harnesses can become dislodged from the webbing. In a few instances, the aluminum seat plate has separated from the harness and fallen to the ground, creating a potential dropped object hazard. There have been no reports of injuries or accidents associated with this condition. The performance of the harnesses is unaffected by this issue—they will perform properly in all respects, including as body support for a personal fall arrest system in the event of a fall, even without the added comfort offered by the seat sling.”

SAFETY ADVISORY – MSA Green Colored V-Gard® Protective Caps
MSA is issuing this Safety Advisory to inform you that MSA has received field reports that some green colored MSA V-Gard Protective Caps are cracking. The cracking occurs at the top of the cap, and is not caused by an impact to the cap. MSA has confirmed that the cause of the cracking is limited to green colored, medium size MSA V-Gard Protective Caps and that corrections have been made to prevent recurrence.

Upon investigation of the field report, MSA determined that the leg strap and chest strap used in select MSA Gravity Welder Harnesses are incompatible. Although the harness can be donned, in the event of a fall, the shoulder straps may extend and affect the protection offered by the harness.

STOP USE NOTICE – Free Standing Counterweight Anchor Using an Evolution Leap Post
3M Fall Protection (formerly Capital Safety) has received field reports of broken end-posts (also known as Lead Energy Absorbing Posts, or “LEAP posts”) used in roof-mounted EvolutionTM Horizontal Lifeline Systems. 3M has launched a global Stop Use – Field Service Action to address this situation. The same Evolution end-posts are also used in Free Standing Counterweight Anchor systems. There have been no reports of fall-related injuries associated with this condition.

INSPECTION NOTICE – CMC Rescue MPD
CMC Rescue has received a very limited number of reports of one or more of the MPD cover plate cap screws (see photo below) found to be loose. CMC Rescue has determined that this condition does not present a safety hazard to users. Per the supplied User Instruction Card, and as with all life safety equipment, we recommend users inspect the MPD before and after each use. If during visual inspection any of the fasteners are found to be loose, contact CMC Rescue to return the unit for service.

The ZorbitTM Energy Absorber is used with horizontal lifeline systems to limit fall arrest forces on the worker in the event of a fall. 3M Fall Protection (formerly Capital Safety) has identified a production issue with its Zorbit Energy Absorbers that creates a risk of serious injury or death for a user in the event of a fall from height. Although there have been no reported accidents or injuries associated with this issue, 3M Fall Protection is voluntarily recalling a limited lot range of this product and will replace affected energy absorbers free of charge.

Lad-Saf™ Sleeve Product Stop Use & Recall / Replacement Notice Immediate Action Required
Capital Safety/3M recently reviewed the performance of the original Lad-Saf sleeve in the field, including a limited number of incidents involving a serious injury or death in the United States while using the sleeve. Although our review did not reveal product hazard or risk scenarios that would arise in the ordinary and proper use of the product, it did reveal potential misuse scenarios that could result in serious injury or death. The potential misuse scenarios include interference with the braking mechanism (such as entanglement with cords, lanyards, clothing or other materials, or grasping the sleeve prior to or during a fall), or result from the user attaching the sleeve upside down (user inversion). No safety regulator has made a finding that the design of the original Lad-Saf sleeve is defective.

Advance Adjustable Offset Davit System (USA version) Product Retrofit Notice
Capital Safety is issuing the attached Product Retrofit Notice regarding the Advanced Adjustable Offset Davit Systems manufactured before January 1, 2016. Capital Safety has determined that DBI-SALA ® Advanced Davit systems do not fully meet some of the loads specified for certain davit adjustment positions as represented in the “Instruction for Use” (IFU) manual and product labels. There have been no reported accidents or injuries related to this issue. Capital Safety has reviewed the design of the davit systems, and has revised all systems manufactured after December 1, 2015 to address these issues.

Capital Safety is issuing a Product Retrofit Notice regarding the UniRail / RoofSafe carriages manufactured before February 2015. Capital Safety has received a limited number of reports of fractured circlips (the locking screw retaining clips) which can allow the locking screw to unscrew and separate from the carriage body. If the locking screw separates from the carriage body, it is possible for the shackle retaining pin to slide out of the carriage body and release the tie-off shackle.
