DBI-SALA® Zorbit™ Energy Absorber – RECALL NOTICE (Note: recall Limited to lot #2101506 through 2464624 only)

The ZorbitTM Energy Absorber is used with horizontal lifeline systems to limit fall arrest forces on the worker in the event of a fall. 3M Fall Protection (formerly Capital Safety) has identified a production issue with its Zorbit Energy Absorbers that creates a risk of serious injury or death for a user in the event of a fall from height. Although there have been no reported accidents or injuries associated with this issue, 3M Fall Protection is voluntarily recalling a limited lot range of this product and will replace affected energy absorbers free of charge.

Lad-Saf™ Sleeve Product Stop Use & Recall / Replacement Notice Immediate Action Required

Capital Safety/3M recently reviewed the performance of the original Lad-Saf sleeve in the field, including a limited number of incidents involving a serious injury or death in the United States while using the sleeve. Although our review did not reveal product hazard or risk scenarios that would arise in the ordinary and proper use of the product, it did reveal potential misuse scenarios that could result in serious injury or death. The potential misuse scenarios include interference with the braking mechanism (such as entanglement with cords, lanyards, clothing or other materials, or grasping the sleeve prior to or during a fall), or result from the user attaching the sleeve upside down (user inversion). No safety regulator has made a finding that the design of the original Lad-Saf sleeve is defective.

Advance Adjustable Offset Davit System (USA version) Product Retrofit Notice

Capital Safety is issuing the attached Product Retrofit Notice regarding the Advanced Adjustable Offset Davit Systems manufactured before January 1, 2016. Capital Safety has determined that DBI-SALA ® Advanced Davit systems do not fully meet some of the loads specified for certain davit adjustment positions as represented in the “Instruction for Use” (IFU) manual and product labels. There have been no reported accidents or injuries related to this issue. Capital Safety has reviewed the design of the davit systems, and has revised all systems manufactured after December 1, 2015 to address these issues.

UniRail / RoofSafe Carriage

Capital Safety is issuing a Product Retrofit Notice regarding the UniRail / RoofSafe carriages manufactured before February 2015. Capital Safety has received a limited number of reports of fractured circlips (the locking screw retaining clips) which can allow the locking screw to unscrew and separate from the carriage body. If the locking screw separates from the carriage body, it is possible for the shackle retaining pin to slide out of the carriage body and release the tie-off shackle.

Capital Safety has learned of broken screw heads found in a limited number of Protecta brand Rebel Self Retracting Lifelines (SRLs). The units affected by this notice are the black, extruded aluminum housing versions of the Rebel, manufactured between 1 April 2014 and 30 November 2014.

DBI-SALA Sayfglida Horizontal Lifeline System (“Sayfglida System”)

Capital Safety is undertaking a voluntary recall of the sleeve (or shuttle) component of the DBI-SALA Sayfglida Horizontal Lifeline System (“Sayfglida System”). The Sayfglida System is permanently installed as a means of anchoring a fall protection system and consists of a Sayflink Sleeve, a wire cable, and by-passable intermediate brackets. The brackets are designed so that the system may be installed above the user, level with the user, or at walking surface level.
